What Is the Difference Between Monopolar and Bipolar Cautery?
Electrosurgical generators cut and coagulate tissue with high-frequency current. There are two basic modes:
Monopolar mode
Current enters through the active electrode and completes the circuit via a return (neutral) electrode attached to the patient. It provides a broad cutting/coagulation field; correct monitoring of return-electrode contact is critical for safety.
Bipolar mode
Current passes only between the two tips of the forceps, minimizing spread to surrounding tissue. Ideal for precise coagulation and small vessels, it requires no return electrode.
Vessel sealing
Modern generators sense tissue in real time, adjust power automatically, seal vessels up to 7 mm and cut energy when sealing completes — reducing bleeding and shortening operating time.
